ATTESTATION OF DOCUMENTS - DOCTORS / NURSES

·     MBBS ·     FCPS ·     PMDC Registration ·     All Experience Certificate from House Job till    to date. ·     Nursing Diploma ·     PNC Card ·      Experience Certificate for Nursing	·   Photocopies of all related documents (one copy each) ·   MBBS Degree duly verified by issuing institute / HEC ·   FCPS Degree duly verified by HEC ·   Attested affidavit of Rs.20 for NRS. ·   Photocopy of NIC (one copy)	·         Application form. ·         Letter of contract. ·         MBBS Degree ·         Valid PMDC registration ·         Valid copy of CNIC. ·         Valid Copy of Passport ·         Fresh Affidavit ·         Valid PMDC registration duly verified by PMDC (Copies of documents attached are required to be attested by Notary Public)	·         Application form. ·         Diploma in Nursing. ·         Diploma in Midwifery. ·         A copy of Metric Certificate. ·         Contract Letter. ·         Experience Certificates ·         Valid copy of PNC Card duly verified by PNC ·         Valid copy of CNIC. ·         Valid copy of Passport ·         03 Recent Photographs ·         Fresh affidavit ·         A permission letters from the concerned Department. (Copies of documents attached are required to be attested by Notary Public)

Note.  

·         All copies of the above mentioned documents must be attested from Gazetted Officer / Notary Public.

·         If documents are submitted by any other person, authority letter from the concerned person to receive the same must be provided).

2.         Schedule for attestation / NOC / Letter of need will be as under:-
